Unilateral Orbital Uptake on Ga-67 Scintigraphy

An 82-year-old woman was admitted to our hospital or evaluation of a mass in the right orbit of 4 months uration. She had no ocular symptoms, fever, sweatng, or weight loss. On physical examination, a 4-cm ass was palpated in the soft tissue of the right orbit. o lymphadenopathy or organomegaly were detected. allium scan showed increased tracer uptake in the ight orbit and in both parahilar regions (Fig 1). omputerized tomography showed a 3-cm mass in the ateral aspect of the right orbit that extended to the etrobulbar region. No hilar adenopathy was demontrated. Biopsy of the mass confirmed the diagnosis of iffuse large cell non-Hodgkin lymphoma. The patient was treated with chemotherapy, and a ollow-up gallium scan, which was performed after 2 ourses of cyclophosphamide, doxorubicin, vincrisine and prednisone (CHOP) chemotherapy, showed isappearance of the right orbital uptake. However, he bilateral parahilar uptake persisted and most likely epresented benign uptake (Fig 2). The patient ompleted 6 courses of CHOP and had complete esponse.

The author reports a rare case of brachial plexus infiltration by non-Hodgkin's lymphoma. T2-weighted MRI showed high signal intensity along the right brachial plexus and soft tissue masses. Ga-67 scintigraphy showed abnormal tracer uptake along the course of the right brachial plexus, and was superior to MRI in detecting the abnormality.
